Undergraduate-Faculty Dinner Registration Form

Welcome to the Undergraduate-Faculty dinner program! After you have gathered 3-6 fellow undergraduates, invited a professor, and chosen a date, please fill out this form to tell us about your dinner.

You may request reimbursement for up to $20/person for as many dinners as our budget allows. We will close this form when we no longer are able to reimburse dinners, so please talk to your professors early to make sure you are able to attend a dinner this term!
